Existing group network application

By filling out this form, you are requesting that MHPN contact you to discuss your existing group becoming an MHPN network.

For example, peer support, interdisciplinary professional development, networking etc
To be eligible for MHPN funding and support, groups must include at least three different mental health disciplines (for example psychologists, psychotherapists, social workers, OTs, GPs, psychiatrists, counsellors etc)

Please note MHPN is unable to support one-off events/meetings.
Please list three times and dates that you are available for a 30-minute introductory meeting with an MHPN Project Officer to explore further if your group can become part of MHPN. Please ensure that the first available time is at least five business days after submitting this form.
