By filling out this form, you are requesting that MHPN contact you to discuss establishing a network.

What is the network's intended purpose?(Required)
While a network’s purpose can change over time, new networks require a clear overarching purpose in order to help determine membership needs and potential meeting topics.
What is your preferred network type?(Required)
A network’s purpose will determine the network type. Local networks bring together practitioners in a particular area to meet in person and/or online. State or national networks bring a wide range of practitioners together to meet about a particular field of mental health (online), and require the partnership of a relevant organisation.
Do you have a group of professionals already meeting, for which you would like to apply for MHPN support?(Required)
Please list three times and dates that you are available for a 30-minute introductory meeting with an MHPN Project Officer to discuss the proposed network. Please ensure that the first available time is at least five business days after submitting this form.
