Kaylene Ryan

Manager and Co-Founder, Australian Disease Management Association

Kaylene is the Manager of the Australian Disease Management Association, which she co-founded in 2005. She has over 30 years of experience in healthcare commencing as a Registered Nurse and has experience in co-design, implementation and evaluation of chronic care and care coordination models.

She has coordinated the development of health system resources including guidelines, a tools directory and national forums in the areas of chronic care and integrated care. She previously worked on the pilot and evaluation of Hospital in the Home across Victorian hospitals which led to their sustained implementation.

Other previous work involves Manager of the Disease Management Unit, Alfred Health; secondments with the Victorian DOH; provision of workshops; seminars on prevention; creator of The Wellness Book Victoria (published 2016). Kaylene is currently focused on coordinating integrated communities of practice and a national webinar series on social isolation and social prescribing. This has led to development of a web platform providing information including Australian social prescribing models. She is currently coordinating a survey in collaboration with stakeholders which seeks to gain a more informed understanding of social prescribing activity. Kaylene is interested in developing systems which bring on clinical support services and health services together to facilitate social prescribing and wellbeing.
